Portfolio Online User Consent Form
INTERNET ACCESS TO ACCOUNT INFORMATION
1. At my request, I have been given access through the Internet to information concerning
my trust account. I have been issued a unique user ID and an initial PIN (Personal
Identification Number). For security purposes, I have been told that the PIN should
be changed promptly to a PIN of my choosing.
2. I have been informed of the importance of keeping my PIN confidential. Country
Club Trust Company has also advised me not to provide either my ID or PIN to any
third party and to notify my trust account administrator as soon as possible if
I have reason to believe that either has been obtained by unauthorized parties or
has been compromised in any manner. I understand that Country Club Trust Company
reserves the right to revoke my PIN and electronic access to the information at
any time.
3. I have been informed of the security measures taken by Country Club Trust Company
and its trust-system provider to protect the confidentiality of any information
provided via the Internet. However, because of the rapidly developing technology
relating to Internet usage, I understand that Country Club Trust Company is unable
to grant an absolute guarantee of the security of account information displayed
on the Internet. I understand that Country Club Trust Company is not responsible
for any breach of security caused by my failure to maintain the confidentiality
of my PIN.
4. I understand that I may print or download account information from the on-line
Access. Unless I am notified by Country Club Trust Company in writing to the contrary,
I understand that any information provided through the Internet shall not be deemed
to supersede or replace monthly or quarterly statements required to be transmitted
by Country Club Trust Company to me. I understand and agree that Country Club Trust
Company shall not be liable for any harm caused by the transmission of information
of a computer virus, or other computer code or programming device that might be
used to access, modify, delete, damage, corrupt, deactivate, disable, disrupt, or
otherwise impede in any manner the operation of any of my software, hardware, data
or property.
5. I elect to receive all trust account statements via internet access and the Portfolio Online
System.
6. Passwords: By system default all passwords are set to not expire.

with being granted Internet access to my Trust Account.
